These recorded reductions in hospitalisation of around 11 per cent. The Australian trial provided important information that contributed to the design and effectiveness of the program. Key factors include: • fast access to 99 per cent of the Australian population • devices that send data automatically without user intervention • Healthways’ nurses using the data to empower members to take an active role in managing their condition • appropriate interactions with GPs to provide them with the data readings • data readings that will be visible to members through the My Health Guardian web portal, with members being able to adjust the notifications they receive by email or SMS. Monitoring and support When a member participates in the telemonitoring initiative, a nurse will set up their individual threshold levels for the tests and device. If a test result is outside of these parameters an alert will be sent to the member and the monitoring nurse, who will contact the member to confirm the readings and advise a course of action. In the future "More than 25,000 members access My Health Guardian to manage a chronic condition. The new telemonitoring initiative has the potential to help a significant proportion of these people live healthier and more fulﬁlling lives,” says HCF’s Managing Director Shaun Larkin. "We expect this ser vice will be provided to around 3,300 members over the next two years."
